Terri Dickens

Terri has experience in a variety of roles, in Eastern Oregon.  She has served as  child Development Specialist, a School Psychologist, and Director of Early Childhood Special Education for 7 counties in Eastern Oregon.   Has been involved in the start up of 4 behaviorally oriented school programs, one that transitioned into what is now Lifeways Day Treatment for Children.   She has experience assessing children from birth thru 21 for Developmental and learning challenges and behavioral conditions and for helping to build plans for success with the child’s team, including parents.   She is experienced in working with a multidisciplinary team,  Spanish and sign language interpreters occupational and physical therapists, teaching staff, speech and language therapists, psychiatrists, mental health therapists as well as professionals from a variety of different agencies in the communities she worked in.

She was clinical director for a mental health clinic and served as the quality Assurance Manager for the same clinic while seeing a full caseload of clients, in order to forgive student loans.  She then worked for Greater Oregon Behavioral Health Incorporated in Care Management, Utilization Management and Integration of Behavioral Health into Primary Care.   She was the Project Director for a Federal Grant promoting Mental Health in schools for Intermountain Education Service District, where she worked for 16 years.

Although she has lived in many places and served in the U.S. Army, Terri was born in Grant County, Oregon.   She is a client centered, strength based therapist, and believes in client voice and choice as well as understanding the myriad of circumstances that affect people’s medical and emotional conditions and the effects on care, making primary team based care a perfect fit for her.

In order to see the Behavioral Health Care Manager, a patient must have established care with the clinic (must have received care in the previous year). The model includes short term interventions for a variety of behavioral and lifestyle issues, referrals to more specialized mental health services when needed and wanted.   We know that most people with a behavioral health concern will receive no treatment, for a variety of reasons.  Having behavioral health care in the clinic allows the care team to address any existing issue, if the patient so desires.  Most patients see the Behavioral Health Care Manager only one or two times for issues that would not traditionally be appropriate for specialty mental health.   These may include, grief, health coaching for diabetes or asthma, sleep issues, after starting a medication for anxiety or depression  or for  child behavioral consults.   The model is more like dental care, in that you may receive a periodic check up to maintain health, instead of waiting until a problem requires intensive treatment. The visits inform the care of the primary care provider, and are conducted at their request, so that care is more coordinated.

Visits and screening instruments are available in Spanish as well as English.   Trained interpreters are available for all appointments.

Here at Columbia River Community Health Services, we are in the beginning stages of developing a support group for local moms. The main purpose of this support group is to be able to provide educational classes focusing on family and personal well-being, also to provide a place for moms to communicate/network with each other.
We are looking for moms to volunteer their time to be a part of the support group and help our clinic in develop a place where moms can get the best benefit out of this group.

We do many lab procedures and almost all x-rays in office. We send some lab tests to Quest Diagnostic Imaging and all x-rays to Blue Mountain Diagnostic Imaging for further analysis.

You will receive a separate bill from them for this service.

They both are happy to bill your primary insurance.

David Collins, PA-C


 Michael Sermon, PA-C

Michael Sermon is a certified physician assistant who joined Columbia River Health in 2015.  Michael was born and raised in Utah, where he completed his bachelor degree in Exercise Science at Utah State University in 2010.  He completed his Masters in Physician Assistant Sciences in May of 2012 at Saint Francis University in Loretto, Pennsylvania.   He then began his career in the Friona, Texas where he practiced family medicine and worked in the emergency room from 2012 – 2015.

He is particularly interested in family medicine because it allows him to be an active participant in the care of his patients through the lifespan.  He firmly believes that not everything can be fixed with a pill, and that the basic principles of regular exercise and healthy balanced diet are vital to good overall health.  He is grateful to serve the community, and looks forward to partnering with patients to help them attain their health goals.

He enjoys spending time with his wife and 3 young children.  He also enjoys the outdoors, hiking, ice hockey, mountain/road biking and playing music in his free time.


 Nathan Lystrup, FNP 

Nathan grew up in Southern California and Las Vegas. His undergraduate was completed at Nevada State College. He worked as a Registered Nurse for 8 years and then Graduated from Regis University with a Master’s degree as a Family Nurse Practitioner.

Over the years Nathan and his wife Mirazz have enjoyed the fun and surprises 6 children can bring to their lives. They enjoy outdoor activities and spending time as a family.

Alysia McDonald, PA-C

Alysia McDonald, PA-C is from rural North Dakota. She completed her undergraduate degree at the University of North Dakota and moved to Oregon with her husband, Christopher, 3 years ago. Alysia graduated from Pacific University in 2017 with her Masters of Physician Assistant Studies and is excited to build her first practice in Boardman.

Alysia and Chris enjoy the many outdoor activities the Pacific Northwest has to offer and are eager to explore Eastern Oregon with their dog, Prudence. Alysia also has a passion for art and enjoys working in various mediums. The rural and hard working agricultural community was what drew Alysia and Chris to Boardman.


Austin Higginson, PA-C

Austin was born and raised in Utah. He and his wife enjoy trying to keep up with their energetic and infinitely curious children. The Higginsons love the outdoors and spend as much time as possible exploring nature. Austin graduated from the University of Utah with a Bachelor of Arts degree in Biology. His time providing care to the residents of the William E. Christofferson Salt Lake Veterans Home as a CNA and restorative aide for over two years inspired him to continue his education in physician assistant studies at Pacific University, Oregon. He is thrilled to join Columbia River Health and is passionate about helping patients improve their lives through healthy choices.
