Your First Appointment
New patients should bring their current medications and insurance card. If you have records or test results from other physicians, please bring them, as they may assist in your treatment and care. Payment arrangements will be handled on an individual basis. Please ask about our sliding scale discount, if you feel you qualify.
We respect the value of your time by keeping appointments as scheduled. If you are unable to keep your appointment, please call us so we can properly treat other patients. Urgent care and walk-in patients will be accessed and fit into the schedule as determined by the medical staff.
Our fees are similar to other health care providers in our area. Discounts are available to eligible patients and are made possible by a grant from the Health Resources & Services Administration (HRSA) – Bureau of Primary Health Care. We must collect a part of the cost of your health care from you on a sliding scale schedule. Please ask about low cost health care to see if you qualify. We accept assignment of benefits from Medicare, Medicaid, and private insurance. We are a preferred Oregon Blue Cross/Blue Shield provider.
We ask that all patients pay their copay, deductible, or any amount not covered by insurance at the time of service. As a courtesy we will bill your insurance for you. Your insurance is a contract between you and the company, and it is your responsibility for any balance owing. Please contact our office billing department if you need assistance. We also have an Insurance Exchange Application Assister on site to help you enroll in Medicaid or Insurance, if you are uninsured.
We will bill your insurance company for you but we ask that you pay any co-payment at the time of service. We will send you a statement every month. If your insurance company has not paid within 60 days we advise you to contact them directly. We can arrange a payment schedule, and we accept Visa and MasterCard.
Occupational Health Services
If you have a business and are interested in our occupational services, such as pre-employment, post-accident and random drug testing, and Department of Transportation (DOT) physicals please call our occupational medical assistants at: (541) 481-7212 x 224 or (541) 481-7212 x 223.
Emergencies take priority, and we request your patience when this occurs. If you have an emergency after hours or on the weekend, you should go to the nearest Emergency Room.
